骨髓抑制，或称骨髓功能抑制，是一种化学治疗及使用某些影响免疫系统的药物，如硫唑嘌呤，所引起的副作用。在化学治疗白血病的风险尤其高。 骨髓是红血球生成的地方，但这个过程会因骨髓抑制而减慢或停止。由于体内不能产生白血球抵抗入侵的细菌及病毒，会很容易引致有生命危险的感染。而且由于缺乏红血球及血小板，亦有导致贫血及严重出血。 因硫唑嘌呤引起的骨髓抑制可以转用其他药物来治疗，如移植器官时改用霉酚酸，或在类风湿性关节炎或克隆氏症改用其他疾病改变药物等。因抗癌化学治疗而造成的骨髓抑制则较难治疗，在感染初期就须留院进行严格的感染控制和使用静脉注射抗生素。
